ABSTRACT

ObjectiveTo explore the anti-gout effect and mechanism of Derris eriocarpa extract by network pharmacological analysis combined with in vivo and in vitro experimental verification. MethodThe chemical components and candidate targets of D. eriocarpa were obtained from the database. The key targets and potential active components of D. eriocarpa in the treatment of gout were screened by the protein-protein interaction analysis, and then the Gene Ontology (GO) functional annotation and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment were performed for the key targets. A mouse model of hyperuricemia was established by intraperitoneal injection of hypoxanthine to observe the effect of D. eriocarpa alcohol extract on hyperuricemia. A rat model of gouty inflammation induced by the injection of microcrystalline sodium urate crystals into the foot and plantar was used to observe the effect of D. eriocarpa alcohol extract on gouty inflammation. A xylene-induced acute inflammation model was established to observe the anti-inflammatory effect of D. eriocarpa alcohol extract. The hot plate test and twisting test were performed to observe the pain-relieving effect of D. eriocarpa. The lipopolysaccharide (LPS)-induced RAW264.7 cells were used to study the anti-gout effect and mechanism of D. eriocarpa alcohol extract. ResultA total of 12 key targets and 15 potential active components were obtained from the D. eriocarpa-component-gout target network. The emodin, betulinic acid, and medicarpin endowed D. eriocarpa with anti-hyperuricemia, anti-inflammatory, and pain-relieving effects by acting on Toll-like receptor 4 (TLR4), NOD-like reception protein 3 (NLRP3), and nuclear factor (NF)-κB. Compared with the control group, the model groups showed elevated serum uric acid level in mice (P<0.01), increased swelling degree of rats (P<0.05, P<0.01), alleviated the auricular swelling of mice (P<0.05), reduced the twisting times of mice (P<0.05, P<0.01), and increased the hot plate pain threshold (P<0.05). Moreover, the model group showed up-regulated mRNA level of TLR4 and protein levels of TLR4, NF-κB, and NLRP3 in cells (P<0.01), and elevated levels of TLR4 and NF-κB in the cell supernatant (P<0.05, P<0.01). Compared with the model group, the alcoholic extracts (20, 10, 5 g·kg-1) of D. eriocarpa lowered the serum uric acid level in hyperuricemic mice (P<0.01), inhibited foot and plantar swelling in rats (P<0.05, P<0.01), down-regulated the mRNA level of TLR4 and the protein levels of TLR4, NF-κB, NLRP3 in cells, and lowered the levels of TLR4, TNF-α, NF-κB, and IL-6 in cell supernatants (P<0.05, P<0.01). ConclusionD. eriocarpa alcohol extract may exert the anti-gout, anti-inflammatory, and pain-relieving effects by regulating the TLR4/NF-κB/NLRP3 signaling pathway.

ABSTRACT

Spike recorded by multi-channel microelectrode array is very weak and susceptible to interference, whose noisy characteristic affects the accuracy of spike detection. Aiming at the independent white noise, correlation noise and colored noise in the process of spike detection, combining principal component analysis (PCA), wavelet analysis and adaptive time-frequency analysis, a new denoising method (PCWE) that combines PCA-wavelet (PCAW) and ensemble empirical mode decomposition is proposed. Firstly, the principal component was extracted and removed as correlation noise using PCA. Then the wavelet-threshold method was used to remove the independent white noise. Finally, EEMD was used to decompose the noise into the intrinsic modal function of each layer and remove the colored noise. The simulation results showed that PCWE can increase the signal-to-noise ratio by about 2.67 dB and decrease the standard deviation by about 0.4 μV, which apparently improved the accuracy of spike detection. The results of measured data showed that PCWE can increase the signal-to-noise ratio by about 1.33 dB and reduce the standard deviation by about 18.33 μV, which showed its good denoising performance. The results of this study suggests that PCWE can improve the reliability of spike signal and provide an accurate and effective spike denoising new method for the encoding and decoding of neural signal.

ABSTRACT

Objective To deeply understand the risk factors of lymphedema for patients with breast cancer after surgery. Methods The phenomenological method was applied in this study. Semi-structured interview was used to collected data from 9 female breast cancer patients with lymphedema after surgery in our hospital from June to September 2016 for generic analysis. Results The risk factors of lymphedema could be categorized into four themes:(1)choice of treatment is the primary cause:axillary lymph node dissection; radiotherapy; chemotherapy; (2)not paying enough attention to lymphedema:lacking the knowledge of lymphedema; imbalance of physical activities for the affected limb; lacking awareness of exercise and protection of the affected limb. Conclusions Axillary lymph node dissection after radical surgery for patients with breast cancer is the primary cause of lymphedema, and paying not enough attention is an important factor, especially lacking the consciousness of prevention, so the nurses should emphasize education about prevention of lymphedema after surgery for patients, to improve the consciousness of them to reduce the occurrence of lymphedema and its influence on their quality of life.

ABSTRACT

OBJECTIVE:To provide scientific reference for rational,effective and economical drug use in pediatric depart-ment. METHODS:A total of 9771 prescriptions were randomly selected from children's branch of our hospital during May 2012-Apr. 2016. Those prescriptions were analyzed statistically in respects of drug type,prescription cost,utilization rate of injec-tion,utilization rate of essential medicine,clinical diagnosis,irrational drug use,use of antibiotics,etc. RESULTS:Among 9771 prescriptions,2.91 types of drugs were used in each prescription,and each prescription expended 77.10 yuan;utilization rate of in-jection was 57.11%,and that of national essential medicine was 67.80%. Respiratory tract disease was main disease (84.17%). There were 284 irrational prescriptions(2.91%),including 156 non-standard prescriptions,165 unsuitable prescriptions and 65 ex-traordinary prescriptions. Utilization rate of antibiotics was 18.25%,among which that of Cefoxitin sodium injection was the high-est(15.48%),but its utilization index was the lowest(0.58). The detection rate of microorganism isolated from bronchitis patients was in low level(45.10%). CONCLUSIONS:The rate of qualified prescription in the children's branch of our Hospital is higher than the requirement of the former Ministry of Public Health concerning the rate of qualified prescription >95%;utilization rate of antibiotics and injection are both in high level. In the future,it is necessary to strengthen prescription evaluation and promote stan-dard and rational use of drugs so as to guarantee safe,effective and economical use of drugs in clinic.

ABSTRACT

Objective To explore the inner feelings of gestational diabetes at their primary use of the real-time blood glucose monitoring. Method Using the qualitative method, 10 gestational diabetic patients monitored their blood glucose using the real-time blood glucose monitoring for the first time and then were interviewed, the acquired results treated with phenomenological analysis. Result The inner feelings of the 10 patients were summarized including 5 themes:worry about the safety of the fetus, fear on alarming at monitoring, different views on four times input of finger blood glucose values, worry about inaccuracy in blood glucose monitoring data, changes in mental conditions and diets. Conclusion Health care workers should provide the gestational diabetic patients with targeted care strategies based on presence of psychological status and knowledge needs so as to improve their compliance, reduce the psychological burden of patients and control their blood glucose level.

Vasculogenic mimicry (VM) is a brand-new tumor vascular pattern with the ability to form vessellike networks without participation of endothelial cells and independent on angiogenesis.It can provide adequate blood supply for tumor growth.The formation of VM involves multiple molecule mechanisms and signal pathways,including cancer stem cell and epithelial-mesenchymal transition.As a unique blood-supply pattern,VM is associated with cancer invasion,metastasis and poor prognosis.Because of its important role in cancer progression,VM will become a new target for therapy of cancers.

ABSTRACT

Objective To investigate the effect of carvedilol combined with spironolactone in the treatment of elderly patients with chronic heart failure clinical efficacy and to provide reference for clinical treatment of formulation. Methods 104 elderly patients with chronic heart failure were randomly divided into observation group and control group,52 cases were randomly divided into observation group and control group.Control group intervention in accord-ance with the conventional treatment of chronic heart failure program,the observation group in the conventional scheme based on,combined with oral carvedilol and spironolactone therapy.6 months after treatment,clinical efficacy was compared between the two groups.The results of echocardiography before and after treatment in two groups were compared.The adverse reactions in the treatment process were compared.Results The effective rate in the observation group was 59.62%,the total effective rate was 96.15%,which were significantly higher than those of the control group (χ2 =4.10,9.82,all P 0.05).Conclusion Carvedilol combined with spironolactone can significantly improve the clinical effect of the treatment of elderly patients with chronic heart failure,effectively improve the left ventricular remodeling in patients with,improve myocardial contractility in patients and has good security.

ABSTRACT

<p><b>OBJECTIVE</b>To study the changes in quantitative kinetic parameters in dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) during radiotherapy and their value for efficacy evaluation in patients with nasopharyngeal carcinoma (NPC).</p><p><b>METHODS</b>Twenty-four patients with NPC that had been pathologically confirmed as poorly differentiated squamous cell carcinoma underwent conventional MRI and DCE-MRI scans 1-2 days before radiotherapy (Pre-RT), during radiotherapy (RT 50 Gy), and upon completion of radiotherapy (RT 70 Gy). Based on the two-compartment model and using the arterial input function deconvolution technique, we calculated the quantitative kinetic parameters of DCE-MRI (K(trans), kep, and Ve) of the tumor tissues, examined the correlation between the tumor regression rate (RS0-50) and the parameters on Pre-RT and RT 50 Gy, and compared the parameters for RT 70 Gy among the groups with different prognosis.</p><p><b>RESULTS</b>The K(trans) value of the tumor tissue decreased after radiotherapy and showed a significant difference between Pre-RT and RT 70 Gy, but not between Pre-RT and RT 50 Gy. The kep value decreased and Ve value increased after radiotherapy. The tumor regression rate was found to be positively correlated with the K(trans) value for Pre-RT (P=0.005) but negatively with the K(trans) value for RT 50 Gy (P=0.001). During the follow-up for 3 years, 5 patients died and 3 patients had distant metastases. No statistical differences in K(trans), kep, or Ve were found between the groups with different prognosis.</p><p><b>CONCLUSIONS</b>The kinetic parameters in DCE-MRI, which vary significantly during radiotherapy, allow monitoring of tumor angiogenesis and vascular permeability and quantitative assessment of treatment efficacy for NPC. K(trans) value for Pre-RT and RT 50 Gy can serve as an indicator for early efficacy assessment of radiotherapy and for treatment adjustment, but its relation with the long-term outcomes awaits further study.</p>

ABSTRACT

ObjectiveTo explore the influence of 3C bolus wizard on postprandial glucose levels in diabetic patients. MethodFifty-eight patients with type 2 diabetes in our hospital were monitored with continuous glucose monitoring system(CGMS), continuous subcutaneous insulin infusion(CSII)and CareLink(3C for short)for 6 days.The function of “3C” bolus wizard was applied during treatment and all the patients were given dietary nursing and health education.The glucose level was observed.ResultThe level of P2hBG of the 58 patients was between 6.4~10.7mmol/L in 3~6 days.ConclusionsBolus wizard plays an important role in “3C” treatment.It can make the postprandial glucose of patients satisfactorily controlled in a short time only to avoid insulin hypoglycemia because of large doses of insulin.At the same time,health education on patients can achieve remarkable results as well.

Objective To explore the effects of Shengmai Injection on tumor growth and the expression of P-gp in transplanted tumor of human gastric carcinoma of SGC7901/VCR cell in nude mice.Methods Transplanted tumor model of human gastric carcinoma of SGC7901/VCR cell in nude mice was built, which was divided randomly into five groups: normal saline control-group, 5-FU group, 5-FU + verapamil group, 5-FU +Shengmai group, Shengmai group. ABSTRACT

Objective To establish a thin layer Chromatograph-Fluores cent method for the determination of karanjin in Radix of Fofdia caulifora. Methods Anhydrous-EtOH is solution, thin-sheet is silica gel, expanding solution is Benzene-EtoAc (8.5∶1.5), with excitation wavelength at 355 nm and emission wavelength at 428 nm. Results The average recovery of karanjin was 99.14% and RSD was 1.94% (n =5). There is a good linear relationship within the range of 0.138~0.69 ?g of karanjin. Conclusion The method is convenient, sensitive, accurate and reproducible, and can be used the quality control of the Radix of Fofdia caulifora.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the response rate and adverse reactions of xeloda, an analogue of 5-fluorouracil, in the treatment of relapsed and metastatic breast cancer.</p><p><b>METHODS</b>Twenty-two breast cancer patients who had recurrent and metastatic measurable foci were treated from Dec. 1999 to Feb. 2000. Xeloda was given, as a single drug, at a dose of or 2,510 mg/m2/d, bid, for two weeks followed by one week rest as one cycle, at least for one cycle in each patient.</p><p><b>RESULTS</b>Among these 22 patients, there was no complete response. Rates of partial response 8(36.4%), stable disease 10(45.5%), progressive disease 4(18.2%), and clinical benefit response (CR + PR + SD) 18(81.8%). The response rate in patients who had failed in previous chemotherapy of taxanes and/or anthracycline was 30.0%-33.3%. The common adverse reactions were hand-foot syndrome, skin pigmentation, nausea, vomiting, anorexia and fatigue. Mild-moderate anemia and leukopenia were observed in 36.4% of patients. Stomatitis, dizziness, diarrhea and chest distress were present in some. One patient developed degree IV myelosuppression. Total bilirubin and alanine transaminase (ALAT) mild elevation occurred in a few patients.</p><p><b>CONCLUSION</b>Xeloda is an effective drug in the treatment of patients with relapsed and metastatic breast cancer, especially for those who have failed in chemotherapy with taxanes and/or anthracycline. Xeloda is well tolerated but has mild adverse reactions.</p>
